Title of Study: Gender and Health.

Introduction: You are invited to participate in a research project being conducted by Drs. Ronald F. Levant at the University of Akron, Seth Pardo at the San Francisco Department of Public Health, Colton Keo-Meier at the University of Houston, and Kate Richmond at Muhlenberg College.

Purpose: The purpose of this research project is to investigate the relationships between gender, ideas or beliefs about gender, health behaviors, health outcomes, and expectations about outcomes of health behaviors.

Procedures:  You may participate in this study if you are currently at least 18 years of age. Participants will participate in the study online, beginning with this informed consent document. Participants who agree to participate will complete a demographic questionnaire and six to seven other questionnaires, for a total of between 122 and 151 questions, depending on the version of the survey you are taking. We anticipate that the study should take about 30-45 minutes to complete.

Incentives:  We offer participation in a raffle for $50, for which there will be four winners.

Risks and Discomforts: Since some of the questions concern gender and sexuality, it is possible that you may feel slightly uncomfortable answering the questions. In the unlikely event that you may become upset in responding to the surveys, please feel free to contact the investigators: Dr. Ronald Levant (levant@uakron.edu; 330-972-5496); Dr. Seth Pardo (Seth.Pardo@SFDPH.org; 415-437-6293); Dr. Colton Keo-Meier (ckeo-meier@uh.edu; 713-743-8600 ext 1), and Dr. Kate Richmond (krichmond@muhlenberg.edu; 484-664-3432).

Benefits: There will not be any direct benefits to you for taking part in this study, other than research credit (for students at the University of Akron) or entry into the raffle.  However, by participating in this study you will help aid our understanding of the factors that influence health.

Right to Refuse or Withdraw: Participation in this study is entirely voluntary and you may refuse to participate or discontinue answering questions at any time without penalization.

Anonymous and Confidential Data Collection: Data collection will be entirely anonymous.  Upon completion of the survey, you will be directed to another website where you will enter identifying information to either enter the raffle or obtain course credit, but the researchers will not be able to link that information to any particular survey responses, maintaining confidentiality.

Confidentiality of Records: All responses to measures will be confidential and will not be traceable back to any individual. Records of this study will be kept private. Any published results will not contain personally identifiable information and your name will not be associated to any of the data.

Who to Contact with Questions: If you have any questions about this study, you may contact the investigators, Dr. Ronald Levant (levant@uakron.edu; 330-972-5496); Dr. Seth Pardo (Seth.Pardo@SFDPH.org; 415-255-3678); Dr. Colton Keo-Meier (ckeo-meier@uh.edu; 713-743-8600 ext 1), and Dr. Kate Richmond (krichmond@muhlenberg.edu; 484-664-3432).  This project has been reviewed and approved by The University of Akron Institutional Review Board.
